• Child's Medical Statement

• Child Enrollment and Health Information Form *updated

• 25-26 Policy Agreement (Please read the Parent Handbook and sign the policy agreement form)

• Additional forms required for students with health conditions, allergies, or food restrictions. Please contact the office for additional forms.

• A copy of certified Birth Certificate is required for Kindergarten students.
